These terms cover your use of the Eshkol Connections LLC website. They do not replace the service agreement you sign if we go on to provide care, and nothing here changes your rights under Colorado or federal law.
Using this website
You are welcome to browse this site, download our forms, and contact us. Please do not attempt to disrupt the site, access areas you have not been given access to, or use anything here to mislead people about who we are or what we provide.
Information on this site
We work to keep the descriptions of our services, service areas and eligibility accurate and current. Even so, this site is general information rather than advice about a particular person's situation. Whether a specific service is available to a specific individual depends on their circumstances, their funding or waiver, and the counties we are serving at the time. Talk to us and we will tell you plainly what applies to you.
No care relationship is created here
Sending a form, downloading an application, or emailing us does not by itself create a care relationship, guarantee placement, or reserve a service. Care begins only after we have spoken with you, completed the required assessments and paperwork, and both sides have agreed in writing.
Applications and the information you send us
If you apply to become a host home provider, the information and files you submit must be truthful and complete. Certification requires background screening and a home study, and we may decline or discontinue an application at our discretion or where the law requires it.
Please do not send Social Security numbers, medical records or financial account details through this website. Sensitive information is collected securely later in the process, directly and through appropriate channels.
Emergencies
This website is not monitored around the clock and must never be used to report an emergency. If someone is in immediate danger or needs urgent medical help, call 911.
Links to other sites
Some pages link to organisations, agencies or resources we do not control. We include them because they may be useful, not as an endorsement, and we are not responsible for their content or their privacy practices.

The text, images, logo and design of this site belong to Eshkol Connections LLC or are used with permission. Please do not reproduce them for commercial purposes without asking us first.
